Learning from accidents
It's all about people
How we learn from surprises, novel situations, turbulences, failures and accidents is largely dependent on how we relate with people.
Do we see people as hazards and objects, or do we really see them as equal others?
Most of us relate with people in the i-it. We assume and tell and the other person does the same. But there’s another way to relate with people in the -thou where we ask open questions and listen so the other person will listen and ask. This is a person oriented way of learning and living.
Our learning from accident workshop is based on the Social Psychology of Risk.
You will learn to understand how human decision making is influenced by the social context and why we need to think in relationships to learn from events.
We will focus on:
- The bias of the investigator
- decision-making and sensemaking
- how social arrangements affect decision making
- Culture of the organisation and,
- Human contributing factors to events.
Learning through experience
As part of this workshop, we will go on an experiential (semiotic) walk. During this walk, we will learn how to observe the environment, how space influences our behaviour, how our hidden beliefs constrain our ‘vision’, and most importantly, how to appreciate and learn from our differences.
A method to understand fallible humans
You will receive a training manual, a pocket guide and a set of tools and methods to apply learning in practice. All our tools and methods are based on Social Psychology of Risk.

Learning outcome
By the conclusion of the program participants will:
- Better understand themselves and how their own assumptions and biases influence event investigation and learning.
- Develop skills in causation and understanding causation bias.
- Learn about the foundations of social psychology and its contribution to event investigation
- Study examples of accident cases (including the Costa Concordia accident) and learn to understand how accidents are narrated and framed.
- Develop skills in perception and defensiveness detection.
- Learn skills of temperament type and engaging others in investigations.
- Understand how culture impacts on events and investigation.
- Develop effective questioning skills and interviewing techniques that support a social psychological approach to event investigation.
- Be equipped with a range of SPoR tools to better manage event and incident investigations.
- Develop strategies to incorporate social psychological perspectives in report writing

Nippin AnandFounderNippin has a Masters degree in Economics, a PhD in Social Sciences and a fascination in Cultural Anthropology. Working as a seafarer and travelling the world from the age of seventeen gave Nippin the unique opportunity to meet people who think differently. This experience has informed much of his work to date, encouraging him to find deeper meaning, as well as learn and grow through every challenge he undertakes.

Pedro FerreiraChief ScientistPedro applies a wide diversity of industry and academic research experience to the understanding of how planning, learning and decision-making are at play in different organisations. He has over 20 years of experience as a consultant and researcher in the areas of Safety, Resilience and Ergonomics.
Pedro holds a PhD in “Manufacturing Engineering and Operations Management” and a master degree in Ergonomics and Health & Safety.
