A step by step approach to improving risk assessments
In many accidents – process failures or personal injuries – here are some typical patterns:
– risk assessment was carried out.
– toolbox discussion was conducted.
– All paperwork was duly filled and filed.
But here are some recurring problems:
– the hazards were identified by someone in the headquarters.
– the controls were determined by the expert who led the toolbox talk.
– the expert spoke and everyone quietly listened. There was no engagement and no questions were asked at all.
People get injured and process failures occur not despite but because of risk assessments and toolbox talks.
In this workshop, you will learn a step by step approach to turning paper-based risk assessments into a visual and interactive exercise of listening, learning and improving decision making.

How to identify confusions, workarounds, conflicts and trade-offs in decision making and risk
Session 6. How to create comprehensive visual maps of risk
A comprehensive exercise on creating visual maps for high-risk operations

What if I cannot join all the six sessions?
You will receive (password protected) recorded videos of all the six sessions, worked examples of risk assessments and incident investigations, and a guidance manual to aid your learning.
And you will become part of a growing community from across industries and geographies that meets online every month to learn, share and practice visual mapping.
